Asus EN210 fanless video card. Low-buck, but now playes HD instead of stuttering/hanging. CPU down to ~8% instead of 60%.
Microsoft Media Center remote/receiver since most rarely want to use the kb/mouse after it's set up.
HD HomeRun networked dual HD/SD OTA tuner
Hauppauge PVR-500 dual analog tuner, although analog-only tuners are generally no longer needed. Some people are using combo analog/digital cards like Hauppauge WINTV-HVR-1800 analog/HD/SD tuner.

This setup takes OTA signals from an attic antenna and analog signals from cable.
